Origin
The lateral and medial cutaneous branches arise from the dorsal branch of the subcostal artery.
Course
The medial cutaneous branch follows the medial branch of the posterior ramus of the corresponding thoracic nerve. It travels posteriorly, crossing the transverse processes of the adjacent vertebrae. It courses behind the longissimus and spinalis muscles and piercing the latissimus dorsi muscle to become superficial.

Along its course, the medial cutaneous branch of the dorsal branch of the posterior intercostal artery supplies the longissimus thoracis, spinalis, and skin adjacent to the vertebral column.
