The proper plantar digital branches of the medial plantar nerve arise from the common plantar digital branches of the medial plantar nerve. The medial-most proper plantar digital branch is a direct continuation of the medial plantar nerve itself.

The proper plantar digital branches of the medial plantar nerve extend from the heads of the metatarsals distally into the toes.

The proper plantar digital branches of the medial plantar nerve provide sensory innervation to the skin and joints of the medial side of the great toe, and the adjacent surfaces of the great and second, second and third, and third and fourth toes. It also supplies the tips of the toes and the nail beds.
